PMDD is under diagnosed. Frequently women are diagnosed with psychosis, personality disorders, bipolar or hypomania. This means sufferers can end up medicated with Lithium and other antipsychotic medication which makes them put on weight, have less energy and loose interest in life which are already distressing symptoms of PMDD.
PMDD is worsened by additional oestrogen and fat cells secrete a form of oestrogen called oestrone/estrone. This is less potent than the oestrogen secreted by the ovaries, oestrodiol, but it can have disastrous consequences for someone with PMDD. Often sufferers are put on the pill which in some cases makes the symptoms worse and can lead to suicide attempts and self harm.
Trans men who experience PMDD pre transition may be mistakenly refused testosterone because testosterone is associated with aggression. Because of the anxiety and other personality changes in the premenstrual phase people experiencing their gender dysphoria may be significantly worse in the pre menstrual phase. People with PMDD are often LOW in testosterone and the aggression, anger and anxiety often arise from unopposed oestrogen so testosterone may significantly improve symptoms of PMDD and associated worsening of gender dysphoria.
